Telling the city when you change address:
when moving out of Fukuoka City to another city, town, village or country


Summary

When you move out of Fukuoka City to another city, town, village or country, you must submit a change of address notification to the city to tell them that you will change your address.
After you have submitted the notification, you will receive a change of address certificate to submit at the municipal office of your new address.


How to notify the city

You can notify the city of your moving out by post or in person.


When to notify the city

You must notify the city when you move out of Fukuoka City to another city, town, village or country.


Who can notify the city

・The person who is moving out of Fukuoka City or the head of their households; or
・Representatives* of people who are moving out Fukuoka City.
*Representatives must be given a letter of authorization from the person who is moving out of Fukuoka    City.
*There is no fixed format for the letter of authorization. The person authorizing the representative must    write the following information on letter paper:
(1)the date the letter was made;
(2)what the representative has been authorized to do (e.g., I authorize this person to XXX on my
    behalf) and why they have been authorized to do so (e.g., because I am too busy to go myself);
(3)the name and address of the representative; and
(4)the name, signature and address of the person authorizing the representative.


What documents you need

When notifying the city by post

You must post the following 3 documents to the Citizen Section of the ward office or branch office of the address you are moving out of. After your documents have been processed, you will be sent a change of address certificate. You can only receive this certificate at the address you were registered at in Fukuoka City, or your new address.

1.A completed moving out of a municipality notification form (postal use only)* ⚠ Only available
    in Japanese
    *You can download the form or collect it from your local ward office.
    *You can also send the necessary information on letter paper.
    *You must provide a telephone number that you can be contacted on during the day.
    *The moving day is the day that you left your address or the day you plan to leave your address.

2.Copy of proof of identification* (residence card, passport, health insurance card, My Number card etc.)
    *The copies must be provided in the same envelope.
    *If you are using your My Number card as proof of identification, only provide a copy of the front side.

3. A return envelope* (write your name and return address on the envelope and attach a stamp)
    *After your documents have been processed, your change of address certificate will be sent back in
      this envelope.
    *If you request a special procedure when moving out (only for people with a My Number card or Basic
      Resident Record card), you do not need a return envelope.
    *If you are moving abroad, or have already moved abroad, you do not need a return envelope.
 

When notifying the city in person 

1.Change of address notification form (for moving out of a municipality)
2.Proof of identification of the person submitting the notification in person (such as
    their residence card, passport, health insurance card etc.)
3.A letter of authorization (if a representative is submitting the notification)
