How to apply for and collect your My Number card


What is the My Number (individual number) card

・Anyone living in Japan for more than 3 months, whether a Japanese citizen or foreign
 citizen, is assigned a My Number.
・The My Number is a number that is used to receive payments and services for older
  residents or when you are ill, to decide the amount of tax you pay to the municipal or
  national governments, and to find out someone’s information quickly during a natural
  disaster.


What is the My Number notification card

・When you register as a resident in Japan for the first time, the government will send a
  letter containing a My Number notification card to your address. Your My Number will
  be written on that card
・ If you have not received a My Number notification card, you can find out your My
  Number by requesting a residence record from your local ward office.


Making your My Number card

Once you have received your My Number notification card, you can make your My Number card. With your My Number card, you can do the following:

・ Get official documents, such as your residence record or personal seal registration
    certificate, at convenience stores instead of at the ward office.
・ Use it in Japan as proof of identity or address.
・ If you have an alias written on your residence record, your alias will also be written
  on your My Number card, so you can use it as proof of your alias.
・ Use it as proof of your My Number at your workplace.

You can apply for a My Number card by any of the methods below.


How to collect your My Number card


Once your My Number card has been made, an issuance notification will be sent to your registered address. Take the below items to the Citizen Section of your local ward office or branch office to collect your My Number card.
・ Issuance notification (the letter telling you your card is ready)
・ Your residence card or special permanent resident certificate that has your
  photograph on
・ Your My Number notification card (if you have one)
・ Your Basic Resident Registration card (if you have one)
