Information on Proof of Vaccination against COVID-19 (as of January 4, 2023)

Fukuoka City can only issue proof of vaccination against COVID-19 to residents registered in Fukuoka City when they were vaccinated according to the Japanese Immunization Act.
If you moved into Fukuoka City after vaccination, you must contact the municipality where you were registered at the time of your vaccination to apply for proof of vaccination.
If you received vaccines abroad, you cannot apply proof of vaccination from Fukuoka City or via the application, because you were not vaccinated under the Japanese Immunization Act. Contact the country or region where you were vaccinated to inquire.

Proof of vaccination for domestic use

You can use either of the following (1) or (2) to prove your vaccination status.


(1)Vaccination Ticket & Certificate of Vaccination for COVID-19 or Record of Vaccination for COVID-19




You can prove that you have been vaccinated with your Vaccination Ticket & Certificate of Vaccination for COVID-19 OR Record of Vaccination for COVID-19 (with a sticker attached inside the red frame, indicating vaccine lot after vaccination).

⚠ The wording 臨時 [temporary] marked on the 1st and 2nd Vaccination Ticket & Certificate of Vaccination for COVID-19 refers to a vaccination category under the Japanese Immunization Act. It does not mean that there is a separate official vaccination certificate.


(2)Certificate of Vaccination for COVID 19 with two-dimensional code (for domestic use)




Certificates for domestic use with two-dimensional codes can be issued in two formats: digital or paper.
If you have a My Number Card, you can get a Certificate of Vaccination from the smartphone application and save it on your cellphone.
If you do not have a My Number Card, you can apply for a Certificate of Vaccination with a two-dimensional code by applying online or by post to the city.

The following document is a Vaccination Certificate of COVID-19 (for both international and domestic uses).
⚠ Some countries may specify a certain type of certificate. Be sure to check the Ministry of Foreign Affairs overseas safety website to confirm which type you need. (Japanese only)





Getting a certificate using the smartphone application

You can get an electronic version of certificate using your My Number Card via the Digital Agency’s official smartphone application.

⚠ If you moved into Fukuoka City after your 1st and 2nd doses of the vaccination, select your address at the time of vaccination to get your Certificate of Vaccination.

Getting a certificate at a convenience store

Since July 26, 2022, you can get copies of your Vaccination Certificates at convenience stores.
For details, check the Ministry of Health, Labour and Welfare website. (Japanese/English)

For domestic use
You will need:
1)your My Number Card and 4-digit PIN Number*; and
2the Vaccination Certificate fee (\120)

For international and domestic use
You will need:
1)your My Number Card and 4-digit PIN Number*;
2)the Vaccination Certificate fee (\120); and
3a Vaccination Certificate of COVID-19 for international use obtained on or after July 21, 2022 at a municipal office or through the application. The passport number on the certificate must match your current passport number.

*The PIN is the four-digit number set when you received your My Number Card at the municipal office.
⚠No refunds can be made after issuance, except for printing defects. Check your vaccination records before applying for a certificate. If you find any errors, contact the Fukuoka City COVID-19 Vaccination Ticket and Certification Call Center. For refunds due to printing defects, contact the convenience store that issued the certificate.
⚠You must check the contents of the certificate yourself before it is issued through the terminal at a convenience store. Bring any relevant documents, such as the Vaccination Ticket & Certificate of Vaccination for COVID 19 that you received at the time of vaccination, and it will be easier for you to confirm the content.

Getting a certificate by online application

Vaccination Certificates obtained through online application can be issued in two formats: a PDF or a paper version.


Getting a PDF certificate by online application

Complete your application through the website below. You will receive an email to the email address you registered (titled 対応完了のお知らせ) within roughly 3 business days to notify you of completion. Log in to the application details page using the URL in the email.
Then, download the ZIP file with the password* from the application details page. Enter the password*, and you can view your Vaccination Certificate.
*The password is included in the notification email.

You can apply by post to receive a paper version of your Vaccination Certificate of COVID-19.
Enclose the required documents together with a self-addressed stamped envelope, and send it by post to the address below.
It will take approximately 1 week to issue a paper version after applying.

Getting a Vaccination Certificate of COVID-19

Apply using the method below to get a vaccination certificate if:
-you have lost your Certificate of Vaccination for COVID-19 or your Record of Vaccination for COVID-19; or
-you have a Record of Vaccination for COVID-19 but need a Certificate of Vaccination for COVID-19 issued by Fukuoka City.
You can only apply if you were registered as a Fukuoka City resident at the time of vaccination.

*It will take approximately 2 weeks for to issue your certificate. If you are in a hurry, apply for a Vaccination Certificate of COVID-19 (a.k.a. vaccine passport).
*If you were only registered as a Fukuoka City resident at the time of your 1st or 2nd vaccination, but you have moved out from Fukuoka City, only a certificate for the relevant vaccination can be issued.
