Information on the COVID-19 Vaccine 1st and 2nd Dose (Updated March 6, 2023)

Fukuoka City is delivering a COVID-19 vaccination program to ensure that every resident who wishes to get a COVID-19 vaccine can do so with peace of mind.
*We will announce information on our vaccination program for after April 2023 as soon as the national government releases details.

  • The Fukuoka City 1st floor lobby is open for COVID-19 vaccinations on Thursdays, Fridays and Saturdays. You can get vaccinated either with or without booking an appointment. You must bring your vaccination ticket.

Who can get vaccinated and which vaccines are used

Who can get vaccinated?

Everyone who is aged 12 or over can get vaccinated.
You must leave a 3-week interval between your 1st and 2nd doses.


Information for men aged 10-29:

On October 15, 2021, the Ministry of Health, Labour and Welfare reported that, although the risk of rare heart inflammation condition such as myocarditis that occur in men aged between 12 and 29 after receiving a vaccine from Moderna is still lower than the risk of developing such condition when infected with the COVID-19 virus, it is higher than after receiving a vaccine from Pfizer. Hence, the Ministry has announced that, for men aged between 12 and 29 only, it is possible to receive a Pfizer vaccine as your second dose even if your first vaccine was from Moderna.

Type of vaccine

The type of vaccine that was first introduced against the COVID-19 is used. In Fukuoka City, the Pfizer vaccine is used.
*The omicron vaccine will be used after the 1st and 2nd doses of the COVID-19 vaccine have been administered. You can get your 3rd dose after a 3-month interval after having your 2nd dose.

Vaccination ticket (for 1st and 2nd dose) (ages 12 and over)

Fukuoka City has sent vaccination tickets to all registered residents aged 12 or over.
If you didn’t receive a children’s vaccine and have turned 12, you will be sent a vaccination ticket approximately one month after your 12th birthday.
If you have moved to Fukuoka City, been vaccinated overseas or in a clinical trial, or lost your vaccination ticket, please go to the How to get your vaccination ticket section of the website for more information.



Vaccination site (for 1st and 2nd dose) (ages 12 and over)

The Fukuoka City 1st floor lobby (1-8-1 Tenjin, Chuo-ku) is open for COVID-19 vaccinations on Thursdays, Fridays and Saturdays. You can get vaccinated either with or without booking an appointment.
Children’s vaccinations are provided on Sundays.

You must bring your vaccination ticket and proof of identity.
Please read the cautionary notes carefully if you are considering getting vaccinated without booking an appointment.

⚠ Children aged between 12 and 15 must be accompanied by a parent or guardian when getting vaccinated. (Children aged 16 and over do not need to be accompanied by an adult.)
⚠ If a parent or guardian cannot accompany their child to their vaccination appointment, other family members can do so instead, but they must bring a letter of authorization from the child’s parent or guardian.

Letter of Authorization to be completed when an adult other than a parent or guardian accompanies a child aged between 12 and 15 for COVID-19 vaccination. 

⚠ The 1st floor lobby of the Fukuoka City Hall has a pediatrician on site on Saturdays.

⚠ Parking at the Fukuoka City Hall:
・There is no free parking area, so please use public transport.



Cautionary notes for vaccination without booking an appointment

If you are a registered Fukuoka City resident, you can get vaccinated without booking an appointment. If you wish to get vaccinated without an appointment, you must arrive at the venue to check in two hours before closing time.

*You may be asked to wait depending on the number of appointments on the day.
You must bring your vaccination ticket and proof of identity.
⚠ You cannot get vaccinated without your vaccination ticket.
⚠ If you live outside of Fukuoka City but wish to get vaccinated in Fukuoka City for some unavoidable reasons,
you can apply to receive a Certificate of Proof of Notification to Receive Vaccination Outside of Your Municipality (jūshochigai sesshu todokedezumishō). You must take this certificate to your vaccination appointment.

⚠ If you are not registered as a Fukuoka City resident, but need to get your COVID-19 vaccine in Fukuoka City for unavoidable reasons, please be aware you cannot make a booking online. Call the call center once you have completed the procedure of Notification of Vaccination in a Different Municipality (jūshochigai sesshu todoke).

How to get vaccinated in Fukuoka City if you are registered in a different municipality

If you are not registered as a Fukuoka City resident but wish to get your omicron vaccine in Fukuoka City, submit a request to Fukuoka City to apply for your omicron vaccination. After submitting your request, you will receive a Certificate of Proof of Notification to Receive Vaccination Outside of Your Municipality (jūshochigai sesshu todokedezumishō). You must take this certificate to your vaccination appointment.
